Dear Student-Athlete and Parents:

University policy requires all athletes to obtain a health evaluation prior to any participation in the Finlandia athletic program. A complete examination with a doctor's signature must be handed into the athletic department before participation in any practices or games occur. New student-athletes must obtain this medical evaluation from their doctor, returning student-athletes may obtain this evaluation from the athletic trainers.

YOU ARE RESPONSIBLE for obtaining this medical evaluation. The forms to be used are accessible via the links below. The forms that must be filled out are called the Pre-Participation Physical Evaluation, Authorization for Use, and Health History Forms. Bring these forms to your examining physician to complete the medical examination.

I trust you understand the importance of being physically and mentally prepared for your participation as a college athlete. Obtaining a sound medical evaluation is one of the first steps of your preparation. Please do not delay in arranging and obtaining this evaluation. When completed, it can be brought to Jen Nesseth, athletic trainer, when you perform your at school pre-participation physical. The Pre-participation physical will be done the week before your first practice.

You will not be allowed to practice without this record of examination in our files. For some of you, the process may be longer if your physician requires further specialty examinations before you are medically cleared for activity. Take that into account as you seek your initial appointment. Do not wait until the last few days of your summer!

EXPLANATION AND PROCEDURE OF INSURANCE PROVISION

FINLANDIA UNIVERSITY ATHLETICS

The following should be helpful in providing all responsible parties with the information necessary regarding Finlandia University insurance requirements and the procedures to follow after an injury.

The NCAA does not permit colleges or universities, including Finlandia University , to provide medical coverage or pay medical bills or any part of any bills incurred for expenses related to injuries, illnesses or conditions which are not sustained as the direct result of an accident in our intercollegiate athletic program.

Finlandia University requires that all individuals participating in intercollegiate athletics be covered by a primary insurance plan. The coverage must be maintained for the entire academic and athletic year. Student-athletes' will not be allowed to participate in Finlandia University 's intercollegiate athletic program until proof of insurance is provided. International student-athletes must have a primary insurance policy that covers athletic related injuries. In addition to your primary insurance, Finlandia University offers a secondary insurance. It is imperative that as a parent or guardian you find out what your primary insurance company will cover while your son or daughter participates in athletics at Finlandia University . If the insurance company requires pre-authorization it is the responsibility of the student-athlete, parent, or guardian to obtain authorization from your insurance company before medical service can be provided. This means that your son or daughter must understand that they need to call their insurance company prior to any doctor visits, and your primary insurance provider must cover any services that may be provided in the Houghton / Hancock area.

 

It is important that you contact your insurance company before your son or daughter participate in athletics to make sure that all medical services will be covered in case your son or daughter becomes ill or injured. We have found that some claims are being denied by insurance companies for medical services provided in the Houghton / Hancock , MI area. We are not trying to mandate that you receive services in Houghton/Hancock, but we would like to have the option to be able send your son or daughter to a physician / physical therapy in the area. We ask that when you contact your insurance carrier that you ask if a rider can be obtained for more freedom of choice while your son / daughter are away at school. The main local provider is Dr. Bruce Trusock with Portage Health.

Pre-existing conditions are not covered by Finlandia University . Therefore, it is imperative that all injuries are immediately reported to the Certified Athletic Trainer for accurate injury assessment, physician referral (if necessary), and appropriate documentation. Your primary insurance will be billed directly for all services deemed necessary by the physician at the time the service was rendered. Finlandia University is not responsible financially for the athlete's medical bills.

Finlandia University will not be held responsible for the expiration or lapse of health coverage for a student-athlete. It is the responsibility of the family and student-athlete to inform Finlandia University of discontinued coverage and/or changes.

If you do not have medical insurance please contact Jennifer and we can provide you with a list and brochures of local temporary insurance agencies that provide short term affordable policies for student-athletes.

We hope this explains the medical policies and requirements for student-athletes at Finlandia University .
